    Node Density Estimation in VANETs Using Received Signal Power

    Accurately estimating node density in Vehicular Ad hoc Networks, VANETs, is a challenging and crucial task. Various approaches exist, yet none takes advantage of physical layer parameters in a distributed fashion. This paper describes a framework that allows individual nodes to estimate the node density of their surrounding network independent of beacon messages and other infrastructure-based information. The proposal relies on three factors: 1) a discrete event simulator to estimate the average number of nodes transmitting simultaneously; 2) a realistic channel model for VANETs environment; and 3) a node density estimation technique. This work provides every vehicle on the road with two equations indicating the relation between 1) received signal strength versus simultaneously transmitting nodes, and 2) simultaneously transmitting nodes versus node density. Access to these equations enables individual nodes to estimate their real-time surrounding node density. The system is designed to work for the most complicated scenarios where nodes have no information about the topology of the network and, accordingly, the results indicate that the system is reasonably reliable and accurate. The outcome of this work has various applications and can be used for any protocol that is affected by node density

    Adaptive Transmission Techniques for Mobile Satellite Links

    Adapting the transmission rate in an LMS channel is a challenging task because of the relatively fast time variations, of the long delays involved, and of the difficulty in mapping the parameters of a time-varying channel into communication performance. In this paper, we propose two strategies for dealing with these impairments, namely, multi-layer coding (MLC) in the forward link, and open-loop adaptation in the return link. Both strategies rely on physical-layer abstraction tools for predicting the link performance. We will show that, in both cases, it is possible to increase the average spectral efficiency while at the same time keeping the outage probability under a given threshold. To do so, the forward link strategy will rely on introducing some latency in the data stream by using retransmissions.     Excess Loss Model for Low Elevation Links in Urban Areas for UAVs

    In this paper we analyze the link between an UAV and a ground control station in an urban area. This link shows a unique geometry which is somewhere in between the purely terrestrial (e.g., a macro-cell channel) and the land mobile satellite case (LMS). We describe a measurement campaign which reproduces the UAV link conditions and shows how the excess loss is mainly dependent on the elevation angle and fairly independent of the distance. Finally, we propose a simple physical model for predicting the excess loss based on a combination of diffracted and reflected components. Results from this model are in good agreement with the measurements

    Renal abscess after the Fontan procedure: a case report

    <p>Abstract</p> <p>Introduction</p> <p>The Fontan procedure is an intervention that helps to correct single ventricle physiology. There are many known long-term complications of 'Fontan physiology'. However, the occurrence of renal abscess in such patients has not yet been reported in the literature. The first generation of adults has now undergone the procedure and it is necessary to be aware of the long-term outcomes and complications associated with it.</p> <p>Case presentation</p> <p>We report the case of a 22-year-old South Indian man who had developed a staphylococcal renal abscess against a background of xanthogranulomatous pyelonephritis, nine years after Fontan surgery. He presented to our hospital with a high-grade fever of 25-days duration but with no other symptoms. Physical examination identified costovertebral angle tenderness and pedal edema. An ultrasound scan revealed a mass in his left kidney. The results of a computed tomography scan were consistent with a renal abscess. Despite treatment with the appropriate parenteral antibiotics, there was no change in the size of the abscess and a left nephrectomy was performed as a curative procedure.</p> <p>Conclusions</p> <p>The learning points here are manifold. It is important to be aware of the possibility of renal abscess in a post-procedural patient. The early diagnosis of a septic focus in the kidneymay help to prevent the rare outcome of nephrectomy.</p

    Inadequate use of antibiotics in the covid-19 era: effectiveness of antibiotic therapy

    Background: Since December 2019, the COVID-19 pandemic has changed the concept of medicine. This work aims to analyze the use of antibiotics in patients admitted to the hospital due to SARS-CoV-2 infection. Methods: This work analyzes the use and effectiveness of antibiotics in hospitalized patients with COVID-19 based on data from the SEMI-COVID-19 registry, an initiative to generate knowledge about this disease using data from electronic medical records. Our primary endpoint was all-cause in-hospital mortality according to antibiotic use. The secondary endpoint was the effect of macrolides on mortality. Results: Of 13, 932 patients, antibiotics were used in 12, 238. The overall death rate was 20.7% and higher among those taking antibiotics (87.8%). Higher mortality was observed with use of all antibiotics (OR 1.40, 95% CI 1.21–1.62; p <.001) except macrolides, which had a higher survival rate (OR 0.70, 95% CI 0.64–0.76; p <.001). The decision to start antibiotics was influenced by presence of increased inflammatory markers and any kind of infiltrate on an x-ray. Patients receiving antibiotics required respiratory support and were transferred to intensive care units more often. Conclusions: Bacterial co-infection was uncommon among COVID-19 patients, yet use of antibiotics was high. There is insufficient evidence to support widespread use of empiric antibiotics in these patients. Most may not require empiric treatment and if they do, there is promising evidence regarding azithromycin as a potential COVID-19 treatment. © 2021, The Author(s)

    Clinical Significance of Thrombosis in an Intracardiac Blind Pouch After a Fontan Operation

    The univentricular heart after the Fontan operation may have a blind pouch formed by the pulmonary stump or rudimentary ventricle according to the anatomy before surgery. Thrombosis in an intracardiac blind pouch of patients with a univentricular heart is a hazardous complication. Because only a few reports have described this complication, the authors evaluated the clinical significance of thrombosis in an intracardiac blind pouch of a univentricular heart. They performed a retrospective review of medical records from August 1986 to December 2007. Four patients were confirmed as having thrombosis in a pulmonary artery stump and one patient as having thrombosis in a rudimentary ventricle shown by cardiac computed tomography (CT). This represents 1.85% (5/271) of patients with ongoing regular follow-up evaluation after the Fontan operation. The median age at diagnosis was 14.2 years. Two of the five patients were taking aspirin and one patient was taking warfarin when they were identified for the development of thrombosis. None of the patients demonstrated thrombosis in the Fontan tract or venous side of the circulation. Brain magnetic resonance imaging (MRI) showed that three patients had cerebral infarction and one patient had suggestive old ischemia. Three patients with thrombus in the pulmonary stump underwent pulmonary artery stump thrombectomy and pulmonary valve obliteration. One patient with thrombus in the rudimentary ventricle underwent ventricular septal defect (VSD) closure with thrombectomy. Thrombus in a blind pouch could cause systemic thromboembolism despite little blood communication. Therefore, surgical modification of the pulmonary stump and VSD closure of the rudimentary ventricle are required to reduce the risk of later thrombus formation. Clinicians should not overlook the possibility of thrombus in a ligated pulmonary artery stump or a rudimentary ventricle after the Fontan operation, which may increase the risk of embolic stroke for patients with single-ventricle physiology

    The Regulation of Sulfur Metabolism in Mycobacterium tuberculosis

    Mycobacterium tuberculosis (Mtb) has evolved into a highly successful human pathogen. It deftly subverts the bactericidal mechanisms of alveolar macrophages, ultimately inducing granuloma formation and establishing long-term residence in the host. These hallmarks of Mtb infection are facilitated by the metabolic adaptation of the pathogen to its surrounding environment and the biosynthesis of molecules that mediate its interactions with host immune cells. The sulfate assimilation pathway of Mtb produces a number of sulfur-containing metabolites with important contributions to pathogenesis and survival. This pathway is regulated by diverse environmental cues and regulatory proteins that mediate sulfur transactions in the cell. Here, we discuss the transcriptional and biochemical mechanisms of sulfur metabolism regulation in Mtb and potential small molecule regulators of the sulfate assimilation pathway that are collectively poised to aid this intracellular pathogen in its expert manipulation of the host. From this global analysis, we have identified a subset of sulfur-metabolizing enzymes that are sensitive to multiple regulatory cues and may be strong candidates for therapeutic intervention
